Eligibility

- Entries must highlight solutions developed, released, updated, or announced in the applicable calendar year
- Solutions should demonstrate improved connectivity, engagement, coordination, or outcomes across the care continuum
What to Submit
- Narrative: Explain the care connectivity challenge, your solution, and the impact it is designed to create
- Impact evidence: Share metrics showing improved engagement, reduced care gaps, stronger coordination, or outcome improvements
- Technical overview: Outline platform features, integrations such as EHR or telehealth connections, and relevant security safeguards
- Visuals: Include screenshots, workflow diagrams, product imagery, or supporting visuals when available
- Supporting links: Add case studies, testimonials, publications, or press coverage when available
Judging Criteria
- Connectivity impact: Demonstrated improvements in coordination, communication, and care continuity
- Innovation: Strength and originality of the approach to connected care delivery
- Adoption: Real-world use across patients, providers, care teams, or health systems

Submission Guidelines
- Eligibility: Open to companies, startups, products, platforms, and individuals making an impact in healthcare technology.
- Requirements:
- 250-word narrative highlighting achievements and innovation.
- High-resolution logo and at least one image of the solution (product photo, screenshot, or device image).
- Customer/client identity must be included. If you wish to withhold this from public release, notify us in writing at the time of submission.
- Fees: A submission fee applies per category. See this award’s page for pricing.
- Evaluation: Judged on innovation, clinical impact, scalability, design, and contribution to healthcare technology.
- Multiple Submissions: You may enter more than one category.
- Award Timing: Winners announced within 60 days of final submission deadline.
